ReVive Skin & Wound provides mobile wound care for high-risk patients in the Lake Norman area. We partner with home health agencies, providers, and families to manage complex wounds in the home setting while supporting continuity of care and reducing unnecessary hospitalizations.

Advanced Mobile Wound Care

In-home evaluation and treatment for patients with complex, chronic, or non-healing wounds who may have difficulty traveling to a wound clinic.

Chronic & Non-Healing Wounds

Care for wounds that are stalled, worsening, reopening, or failing to improve with standard treatment.

Pressure Injuries

Assessment and management of pressure ulcers, bed sores, and skin breakdown in high-risk or homebound patients.

Diabetic Foot Ulcers

Specialized wound support for diabetic foot wounds, delayed healing, infection risk, and recurrence prevention.

Venous & Arterial Ulcers

Evaluation and treatment planning for lower-extremity wounds related to circulation, swelling, and vascular disease.

Post-Surgical Wounds

Support for surgical wounds, delayed closure, drainage, infection concerns, and wounds that are not healing as expected.

Debridement

Removal of non-viable tissue when clinically appropriate to support wound healing and reduce barriers to progress.

Advanced Modalities

Use of advanced dressings, biologics when appropriate, and evidence-based treatment plans for difficult wounds.

Care Coordination

Collaboration with home health, primary care, specialists, and caregivers to keep everyone aligned on the wound care plan.

When Should You Call?

  • A wound is not improving after 2–4 weeks
  • There is increased drainage, odor, redness, or pain
  • The patient has difficulty getting to a wound clinic
  • The wound keeps reopening or getting infected
  • Home health needs additional wound expertise
